Is radiology and mohs procedure required CLIA number?

Mohs Procedures Requiring CLIA Number

Effective January 1, 2008, ASCs can now bill the facility fee for Mohs procedures. The following codes (not an all-inclusive list) have been added to the ASC approved list:
• 17311.
• 17312.
• 17313.
• 17314.
• 17315.

ASC claims submitted without a valid CLIA number will be rejected as unprocessable. The CLIA number should be entered into Item 23 of the CMS-1500 claim form or the electronic equivalent for correct claim submission.

All Mohs surgical procedures require the submission of the CLIA number and this requirement applies to surgeons as well as the ASC facility.

Radiology Procedures Requiring CLIA Number

Under the new 2008 changes, certain ancillary services are also separately payable to ASC facilities. The following radiology procedures require the CLIA number for processing:
• 78110TC.
• 78111TC.
• 78120TC.
• 78121TC.
78122TC.
• 78130TC.
• 78191TC.
• 78270TC.
• 78271TC.
• 78272TC.
